After reading it, from beginning to end, for me, the story was meaningless. I can't explain more but this is the first time that I can't accept the score of a manga and I dont understand it. Please tell me what did you really like in this manga. And these drawings...
It's a hard manga to digest and follow, at first, you're thrown into this very chaotic world in Kaiman's prespective where you have no idea what's happening, why Kaiman lost his head, why he has no magic and he's resistant to it, the secret behind the hallway etc. It builds up mystery extremely well up til the end where everything gets answered accordingly, story was enjoyable to me and the mystery with each plot thread was masterfully done, but what cut the deal for me was the characters; Dorohedoro perhaps has one of the best character casts I've ever seen in manga, every character in this manga matters and has a purpose, and they're all very likable, we have Kaiman and Nikaido as our first set of protagonists, against the En Family which are our second set of protagonists, each protagonist has a certain motive and personality, and they all develop from start to finish, whilst I was reading the manga I had an inner conflict of who I liked more, and how I really didn't want Kaiman/Nikaido to face En's Family, I didn't know who to root for, and why, because both parties had valid reasons for fighting.
The story is nonsensical and mysterious at first, but it ramps up after we meet our next set of important secondary characters which are The Cross-Eyes, every chapter we have new plot threads to discover and theorize about, I don't think this is a good manga to read from start to finish in one setting or in a couple days, because you seriously need time to digest what's happening in the manga to understand; It's not a manga you can just read from start to finish without much thought, because Dorohedoro wasn't designed to be read that way.
As for the art; yes, it is EXTREMELY gorey, so if you're not into that then you probably shouldn't pick it up, but I found it very well done nontheless, I enjoyed it a ton, and Q Hayashida seriously puts into prespective how pretty her world building can be.
If you felt like you didn't understand the story, then maybe it is wise to reread again but not at once or over the course of a couple days, as it takes time to digest. It's one of my favorite mangas, ever, and for a reason: It has amazing cringeworthy art, phenomenal likable characters, insanely thought-out character plot threads, simple yet complex world-building, and to top it all off quite a bit of comedy here and there to not make it serious all the time.
Take my opinion with a grain of salt because this is seriously one of my favorite mangas of all time, and it's worth a 10 to me,

Thanks a lot Nayonara for your answer, I learned from it. There is a thing in this manga that stopped me, even it was hard enough, its all reference and signs about evil and religion (maybee you can't understand but i'm really sensitive to these kinds of things). But now i understand better this aspect you enjoyed in Dorohedoro. ❤️
Of course, all that matters in the end is your own enjoyment, and it's totally understandable for you to dislike stuff like that. In my experience, I personally found the gore a tad bit too appalling sometimes, as someone who is an atheist, I don't really have any sensitivity regarding religion in manga or media, but there is quite a ton of satanic symbolism in the manga that some readers may not like, which is also fine; like I said, at the end of the day your personal enjoyment of the manga comes first and everything else you can just ignore.
man that's an easy question for me since imo it's simply flawless, the manga is built like a puzzle in a world that you can't even predict since anything can happen.
Dorohedoro is a mindless fun with a surprising depth to it, when all the pieces are connected to each other and the puzzle like story is finally completed that's where you just know that shit's about to be even more chaotic.
I've never experienced a story told any better in the form of a manga
