I just watched the movie but I didn't really get the ending. why didn't Sakuta and Mai recognize

Pretty much this, I'll just add ...
... because of the strong bonds with everybody, even after Shouko disappeared from Sakuta's life, he managed to meet Mai and others (while in the original timeline it was thanks to Shouko). Either for the same reason, or because he also traveled in time, Sakuta was able to recognize Shouko even though he technically never met her. I like to think it's a combination of both.
what I think is that the stronger the connection was with Shouko, the more likely they were to remember her in the new timeline. Mai had a good connection but not as strong as Sakuta, which is why she wanted to do the movie about the girl with a heart condition, but didn't know why. Why Sakuta recognized her is probably because he kept his memories. Before the timeline is changed, it shows that he is going through memories, which might be a sign that he was transferring them. In the new timeline, everything goes the same still because he has those memories, except for him meeting Shoko. He never meets Shoko and he thinks shes just a dream he had. Everything we see happens in the show happens again. Then we finally reach the point where Sakuta sees Shouko, and the old memories flow back thanks to his bond with her.
Again, this isn't canon, but this is just the way I look at it.
As far as I'm aware, Shoko went back into the past to finish her assignment, which gives her a reason to look forward and to see a life outside of being a kid because originally she never finished it. Sakuta wanted to "attempt" to go back in time to save her, but realized that you couldn't do anything to change getting or dying from an illness. So I think she lived by mere coincidence and luck.
However, don't take my word for it. I'm kinda having difficulties understanding that scene too.

Personally, I think Shouko is the only one who still have memory of everything since she's the one who do the rollback in her dream. That's why she looks incredibly happy when hearing him calling her name.
I think this part is wrong because If she stopped her puberty syndrome then she doesn't have that power anymore.
Although She remembered in the end because of the feeling they all had from other timelines.
Shoko (in the ICU) go back to the forth grade and doesn't have her Puberty Syndrome anymore (going back and completing her "Plans for the Future" assignment, which she couldn't the first time). This stops her from being able to time travel (because she wasn't afraid of the future anymore). While everyone retains their memories (Mai wanting the movie part, Sakuta remembering Shoko at the end) from the series and the movie up until that point. Like deja vu/dream; similar to how Nodoka dreamt that Mai died in a car crash.
Everyone still has their Puberty Syndrome struggles which explains how everyone still met each other: Mai, Tomoe. Along with vague feelings from the past timeline which guided them together.
The heart disease situation (which is incurable) would be resolved from a donor and her new found 'will to live' (which has been shown to strengthen you mentally and physically when in a dire situation).
Although it may seem like a weak plot that she miraculously got a donor; the entire series and movie (until Shoko fainted) is another timeline/'Shoko dream world/simulation' which parallels and we meet up again 6 days after Shoko faints in the first timeline/'Shoko dream world/simulation'.
There is no explanation for how she got a donor, but I would like to believe the movie Mai starred in "The Gift of Spring" about a girl with a heart disease waiting for a donor, spread awareness and galvanised people to register as a donor. In effect, saving Shoko's life and giving her a future.

She basically changed the past so that it doesn't affect Sakuta, but he see's her at the end and gets random memories of her because they are on the beach. Just like how Mai-san made a memory of her teaching him Kanji and he starts doing the same problems in school and gets memories of her. That's what I think

After the scene of Makinohara in 4th STD she changed her past and future by answering the paper the teacher gave her. After that the incidents take places are in new timeline where Sakuta never saw Makinohara and Makinohara does not have the heart diesase and Sakuta never had a problem with his heart.
I think Campyfire got it closest to my interpretation of it. Sakuta realizes that he doesn't want either future (himself dying or Mai dying). He then goes to the hospital to see Makinohara, for what appears to be the last time, and tries to explain what he knows and that he will still find a way to help her. BUT, Makinohara then reveals that she also knew everything all along, this is why Sakuta is so shocked. She then says that she, herself, will go back and fix everything (pretty much the same idea that Sakuta had... almost). Sakuta honestly didn't know how he was going to do it, but he wanted to cure Makinohara in the past and make it so everyone lives and he still gets to be with Mai. Makinohara stubbornly explains that she will do it her way. She'll go back in time, stop herself from developing puberty syndrome (by completing her school assignment and not fearing the future), thus making it so the future Makinohara (Shoukou) never meets Sakuta.
What I understood from that - was that she will still die, but Sakuta won't know who Makinohara is and would not have to mourn her loss. At first, I thought that this would also mean that Sakuta doesn't get the help he needed when he was depressed, but then I thought about how all of that happened because of his injury, and his injury doesn't exist if Makinohara doesn't have puberty sydrome. But it turned out that none of that mattered anyway because Sakuta still remembers Shouka as a "dream" and the future happens normally, just excluding the parts with Makinohara. The very ending shows Sakuta and Makinohara meeting again and reveals that both of them have their memories of the other timelines. So it was a happy ending where everyone lived and everything still happened as we saw it before... Still not sure how Makinohara cured herself of heart disease though???
If I remember correctly, near the end Mai mentioned that she felt inspired and played the role of someone with heart disease (or something similar to that) and as a result of her playing that roll a lot of people signed up to be in a list of those who will willingly give away their heart in the case they die prematurely. As a result Shoukou finds someone with a compatible heart OR the long waiting list is lessened due to the wave of new people donating.
Ah this makes more sense, I was thinking her playing that role brought awareness... which it did just my mind was thinking more research side of things that helped medical research advance enough to give her increased odds against her condition. Maybe I missed something
Awareness = increased donors make more sense now in my head.
