Yeah, so in the manga it is explained right away why he had to do it but it was exposition explanation so I get it why they removed it. Basically in their world only infants or people who depleted all their mana reserves get magic sick. The treatment is familiarizing which in case of infants means that the mother is hugging the baby skin to skin, but in the case of an adult it wouldn't be enough. Seiichirou's case is special because he is an otherworlder, so he is completely not adapted to exist in this world full of magic, so that's why the max skin to skin and fluids contact was needed and the easiest and quickest contact like that is basically sex. It was an emmergency sex.

Ascendance of a Bookworm. In-universe, sharing mana with someone else is something that's normally only done as preparation for sex, when a woman has mana problems while giving birth, or when a woman shares her mana with her child as part of breastfeeding. Ferdinand saves Myne at some point by sharing his mana with her. Even though it's not sex and there was no sexual arousal at the time, it's technically being saved with unexpected bedroom play by in-universe logic.
I watched that series but tbh I didn't know about this tidbit. Tho I guess it's still different to Myne's magic sickness case because magic sicknes in Ascendance's world only happens to kids with excess mana due to inability to drain it and most of those kids die if they're born in lower class and never get discovered. Myne was lucky to be discovered and be able to give away excess mana via her priestess training. So even tho sexual implication is there, it's not the only way to give away / share mana while here we have more of a unique case of lack of exposure to magic overal due to not being born in this world. But alas a similar thing to giving away mana via faith does exist in this world too but it's in a later arc and is tackled a bit differently and doesn't pertain to magic sickness. I won't speak about it to not spoil as from the opening I can say that this arc was animated.
So actually,
the bookworm anime hasn't gotten to the point I was alluding to. It's from one of the later volumes of the novel. The mana sickness I'm referring to isn't the devouring. In the bookworm universe, sharing mana with someone means directly from person to person; it's not considered sharing mana when it's given to a magic tool like the temple's divine instruments. I'm looking forward to the things that you mentioned at the end there. It sounds like it might actually be even more similar to bookworm than I thought! I might pick up the novels later if the anime continues to be this enjoyable.
I wonder if they'll explore the concept of homosexuality and the public view of it in the other world, is it accepted, tolerated and/or supported in that world just like in ours or is it frowned upon?

They never discuss homosexuality in society basically. It's never questioned by anyone that their Knight Captain is gay. And tbh I prefer that. Queer people also need media that don't constantly remind us that our queerness makes us "different". It just existing and nobody batting an eye is 100% improvement on real world.
Out of curiosity, does the LN also fade to black or is it more detailed? I understand that's not an option for a tv anime adaptation.

I find it funny that this is exactly how I would write the sex scene lol. Give people some bait and fade to black. Not because I can't write a decent smut but because I'm simply not interested in it unless 90% of that smut is some emotional earthshattering life changing talk lol. You get 90% emotions, 10% sex.
(And I say it as someone who read and enjoyed pure smut as well lol)
The fact that Seiichirou didn't even have a typical reaction for those types of situations like "what have I done" always takes me out (when I read this scene, I always cackle lol it's hilarious OMG). He just wakes up, looks at the clock, and runs because he is late for work. No thought spared to his sexy times with a handsome young man lol. As if a random sexy time with a man was so normal you could basically treat it like an average Tuesday. But what always takes me out with laughter even more is Aresh reaction to him leaving. Completely unprepared. (I wonder if he felt like a one-night stand lol). I must say, tho that it was more satisfying in the manga. But still hilarious!
Speaking of which, they skipped at least one frame from the sex scene (wait no, they actually moved it into another scene lol what a flashback in the office Seiichirou, and ofc he is talking about Aresh not even wanting to have sex while the scene shows them doing the do lol I guess moving this picture here works a bit better due to Aresh facial expression being strained which means we as an audience will trust Seiichirou that Aresh may have not been ok with it - meanwhile he couldn't literally care less lol because he already adopted Seiichirou as his to care for but ofc Seiichirou will just not acknowldge the elephant in the room and will tell himself that it was all jut some kind of magical service he needs to repay Aresh for). They also skipped the exposition explanation why the sex treatment must have happened (i.e. the whole part about how infants in their world get magic sick and they are being familiarized by their mother's skin to skin contact which unfortunately won't work for an adult because they're not little and they need more skin to skin coverage), and they moved Seiichirou thinking to himself that his back hurts to the later scene (there were two scenes when he thinks that, they just skipped the after-sex one and went straight to the second one with the donut cushion lol - I love the donut cushion btw, it's a must for every sitting job, srsly).
Anyway I was waiting for this episode because one of my favourite things in this manga was the fact that they actually touched upon the topic of "What happens when a person from a world with 0 magic gets transported to a world where magic is a norm and exists in everything - how will their body react to such exposure?" because in majority of magical fantasy stories it's a no brainer that character that gets isekaied is 100% ok with magic and often can even cast magic themselves, but here because Seiichirou was not supposed to be summoned, he gets the full blown "you are in the toxic zone" experience. He was lucky Aresh was there to discover his dangerous shenanigans in time before he actually killed himself.
And speaking of Aresh - he is a true man on a mission. His mission, tho is a stubborn otherworlder that needs to be examined by a doctor and monitored 24/7. The marbles will be lost. On Aresh side. (And him taking charge of Seiichirous's health is basically him claiming the guy on day one lol it's his guy to worry about, nobody can make medical judgement over this guy, he's Aresh's - it's hilarious lol and twice as hilarious after you are basically told by every character in the closest vicinity that Aresh usually doesn't care about anyone at all, and yet he did that in the very first episode already by approaching Seiichirou first lol Also people should really explain to Seiichirou that no, it is not punishable offense to trouble a noble, it's just that Aresh specifically is seen as "the prickly dude that would most likely punish someone" by literally everyone because he is usually just aloof and his mere presence scares people lol). Get prepared for a full-blown experience of Aresh nagging wife persona. You will get a whole TV show just about it.
Ah, and the third thing that always takes me out (with laughter ofc) is how everyone around Seiichirou acts as if whatever he said was 100% insane when he talks about his health. While in truth this is literally a common sense reaction to how unhealthy and deadly overwork culture is, but to Seiichirou they're all exaggerating because he lived like that for years and it's normalized in Japanese society to live like this, so he doesn't get what their issue is, lol. While their issue is literally that it's a deadly lifestyle that nobody should be leading. Seiichirou even considering having gay sex for the sake of drinking the tonics and Aresh catching on right away to what he was thinking about as if he was taking care of a drug addict is seriously one of the most hilarious (and worrying) things lol Also yes, they're basically on a dinner date after this; it will keep happening lol. And just saying, everyone, including Aresh, is right. Sei got a basic income and a roof over his head, so he literally is not obligated to do anything; he could have just lazed around all day doing nothing, but due to how much Japan brainwashed him into working culture, he couldn't just take it and rest, no. He came right back to his unhealthy overworking habits and keeps worrying everyone. BTW It was cut out there at the dinner, but Sei basically has a thought process that Aresh doesn't understand because he is a noble, not because working so much to the point of exhaustion, health issues, and possible death is something incredibly abnormal to do. This guy literally wouldn't recognize a work-life balance if it slapped him in the face - that's why he needs Aresh as his nagging wife lol.

You don't even know how many days I dug through internet for isekai gay manga. I love stories that play with themes and concepts of popular genres. It's always so fun to read how author tackled smth differently than most other stories. I must say that Seiichirou dropping unconcious at a drop of a hat all the time was also an incentive.

Sadly there is not many. I found really cool manhwa tho. It's called Eternal Covenant. The plot is very intriguing, and there is magic and dragons. It's not isekai tho.
From isekais the closest to Bean Counter would be Magirevo (yuri isekai) which was already animated and aired not long ago.
Oh wait, there is one isekai manhwa that has a very cool concept and very deep story. It's called "I Have To Be A Great Villain" and it's a multiple isekai as the protagonist keeps jumping between many worlds and each world has different theme and setting but always the same ML to which the protagonist is a villain but ends up his lover each time. And slowly you learn there is far more to his universe hopping than just playing a role of a villain. There is a true intrigue.

BTW Even tho I read many BL for the last 15 years (lol) I had a really cool discovery with Twilight Out of Focus that aired last year I think. It's a school romance about a club that wants to make a BL movie and I loved the manga and the anime. Devoured it!

so sad it seems rating of this one is affected on all sites due to people's inability to read tags/genres.... either way i'm personally loving it!! seichirou's attitude is killing me though (in a funny way)
It happens a lot in non-anime circles, specific targeting usually but also just a backlash towards LGBTQ+ content - I think other BL may have escaped unscathed and have kept positive ratings... and this feels dumb... but perhaps because they do not have Isekai in the title? As other BL can maintain 80+% on AL but "Sasaki and Miyano" does not draw the same audience's attention.
As the season goes on and more BL viewers pick it up I hope the average ratings can move without this deluge of backlash due to missing the tags.
